Add up your subtotals and enter this on the SubTotal line above the Handling Fee section. If ordering by credit card, your shipping cost will be calculated at the time of shipping and billed to your card (do not enter a shipping cost). If you are paying by check, please call 518-398-1432 for the appropriate shipping cost and add this to the form before adding up your Total. Complete the Shipping, Billing, & Credit Card Information (if applicable) sections of the form.
